Written Notification and Parent Consent Forms to Access Public Insurance (Medicaid)

In order to access the School Medicaid reimbursement, a school district or ECSE program must provide parents with a written notification that explains the protections they have related to giving permission to the use of their public insurance (Medicaid). The notification must also include a description of the personally identifiable information that may be shared and must be provided prior to obtaining parent consent. The school district or ECSE program must then provide the written notification on an annual basis thereafter.

In order to support the implementation of Oregon’s recently approved State Plan Amendment (SPA), the ODE worked with OHA, the School Medicaid Core Team, and other School Medicaid partners to update the written notification and parent consent forms. We updated the language in the forms to be more parent and user friendly and they have been written so that they can be used for both the IDEA and Section 504. They have also been translated into 8 languages. The forms can be found on ODE’s School Age Forms web page.
